慢性胰腺炎胰腺纤维化发病机制研究新进展

摘 要:摘 要:慢性胰腺炎(chronic pancreatitis, CP) 是由多种因素引起的胰腺内外分泌功能紊乱,可导致胰腺结构和功能发生不可逆性损伤,是临床常见的消化系统疾病。CP 的病理特点是腺泡细胞损伤导致巨噬细胞等多种炎症细胞浸润,从而分泌大量促炎细胞因子,在胰腺组织微环境中引起胰腺星状细胞活化,进而产生大量的细胞外基质,表现为胰腺纤维化。而新近的研究提示:胰腺纤维化是一种动态病理现象,需要由多种自分泌和旁分泌的细胞因子组成复杂的网络,作用于相应的信号通路,最终导致纤维化形成。现以CP胰腺组织微环境中出现的主要细胞,如胰腺星状细胞、巨噬细胞、腺泡细胞及其在CP 胰腺纤维化进展中的变化和作用为切入点,对CP 胰腺纤维化的发病机制研究进展做一综述。

New insights into the pathogenesis of pancreatic fibrosis in chronic pancreatitis

Abstract: Abstract: Chronic pancreatitis(CP)is a common disease of digestive system in clinic. A variety of factors can cause the disorder of the internal and external secretion of the pancreas, which eventually leads to irreversible damage of the pancreatic function. The pathological feature of CP is the injury of acinar cells in CP. Acinar cell injury can lead to infiltration of macrophages and other inflammatory cells, resulting in secretion of a large number of proinflammatory cytokines in the pancreatic tissue microenvironment to activate pancreatic stellate cells. These activated PSCs can produce a large number of extracellular matrix to induce pancreatic fibrosis. Recent studies suggested that pancreatic fibrosis is a dynamic pathological phenomenon that requires a complex network of various autocrine and paracrine cytokines acting on the corresponding signaling pathways. Based on the role of acinar cells, macrophages and pancreatic stellate cells in the microenvironment of CP, the new research about pathogenesis of CP was reviewed in this paper.
