Abstract:
Sphingomyelin (SM) is a major structural phospholipid in mammalian cell membranes and serves as a central hub in sphingolipid metabolism. SM not only maintains membrane integrity and organization, but also generates multiple bioactive metabolites, most notably ceramide (Cer) and sphingosine-1-phosphate (S1P), which exert pleiotropic and often opposing effects on cell survival, apoptosis, proliferation, inflammation, and immune responses. Increasing evidence has established that disruption of SM metabolic homeostasis is closely associated with the pathogenesis of a wide range of human diseases. SM metabolism comprises de novo synthesis, catabolic degradation, and salvage pathways, coordinated by a series of key enzymes, including serine palmitoyltransferase (SPT), ceramide synthase (CerS), sphingomyelin synthase (SMS), sphingomyelinase (SMase), and sphingosine kinase (SPHK). The activity and expression of these enzymes are regulated through multiple layers of control, including transcriptional regulation, post-translational modification, subcellular compartmentalization, and feedback modulation by metabolic intermediates. Such regulation is essential for maintaining the dynamic balance among SM, Cer, and key downstream metabolites, thereby shaping downstream signaling networks and cellular fate decisions. Accumulating studies have highlighted the pathological relevance of dysregulated SM metabolism in cancer, immune and inflammatory disorders, metabolic diseases, neurodegenerative diseases, and cardiovascular diseases. In these settings, altered sphingolipid flux contributes to aberrant signaling, organelle dysfunction, inflammatory remodeling, and disease progression. Particular attention has also been directed toward the therapeutic potential of targeting critical metabolic nodes, such as SMS, SMase, and SPHK, with several candidate interventions showing promise in preclinical and early clinical studies. Continued advances in lipidomics, structural biology, spatial omics, genome editing, and artificial intelligence-assisted drug discovery are expected to accelerate mechanistic dissection and translational exploitation of SM metabolism. A deeper understanding of SM metabolic regulation may provide novel opportunities for disease diagnosis, prevention, and targeted intervention.
